How to Request Data

If you’re interested in obtaining public use data from the California Behavioral Risk Factor Surveillance Survey (1984 – 2008) or the California Women’s Health Survey (1997 – 2008), please download the appropriate form. Once you sign and fax your agreement, the data will be sent within five business days. Please note that public use data files are available after a one year embargo.
 
Hint: You might want to refer to the technical documentation for the survey to determine which year(s) of data are of interest.
